BLACK CREEK - Authorities responded to a three-vehicle crash in Black Creek Sunday afternoon.
It happened around 3:30 p.m. on Trap Rd. near State Highway 54.

Authorities say a 24-year-old Appleton woman traveling east on 54 had slowed her car down to turn left into a home. An SUV driven by a 17-year-old Seymour boy slammed into the back of the car. The truck then went into oncoming traffic and collided head-on with a van driven by a 63-year-old Weyauwega man.

The Appleton woman was taken to the hospital by ambulance. The Seymour boy as well as the driver and a passenger in the van were airlifted to the hospital.

Two children, ages 1 and 3, were in the Appleton woman's car. They were secured in child safety seats and were not hurt.
